We would like to share with residents the reviews and impressions of the international clinical internship:

 

Abdrakhmanova Azel Khamzievna, Kadylbekova Raushan Baurzhankyzy

Residents of 2 years of study in the EP “Physical Medicine and Rehabilitation for Adults and Children” of NJSC “Astana Medical University”

The clinical internship took place on the basis of the Russian Children's Clinical Hospital - branch of the Federal State Autonomous Educational Institution of Higher Education Russian National Research Medical University named after N.I. Pirogov of the Russian Ministry of Health at 117 Leninsky Prospekt, under the leadership of Andrey Nikolaevich Lobov and Professor Olga Arlenovna Laisheva.

During the entire internship, a large amount of new material was mastered – 2 lectures by Professor O.A. Laisheva were listened to on this topic “New methods of pediatric rehabilitation”, took part in 2 training practical sessions on “Reflex Breathing” for resident doctors, also conducted by Professor O.A. Laisheva. Additionally, we completed the full course “Kinesio Taping”, upon completion of which we received an international certificate.

Familiarization with the operation of new modern devices for the diagnosis and treatment of musculoskeletal disorders in children was carried out. All this was carried out with extensive use of the Voigt, Bobath, and PNF methods. Most often, we encountered patients with motor disorders as a result of complex treatment of patients with neoplasms of the spinal cord and brain, traumatic disease of the brain and spinal cord, consequences of impaired cerebral and spinal circulation, perinatal lesions of the central nervous system, consequences of a previous neuroinfection, cerebral palsy, diseases of the peripheral nervous system, degenerative diseases of the nervous system, posthypoxic encephalopathy, spinal scoliosis. During the internship, he improved and carried out medical rehabilitation of patients in various areas such as: Physical therapy; physiotherapy; robotic mechanotherapy; kinesiotherapy; massage; kinesio taping; speech therapy correction; psychotherapy.

A special feature of the department is the active use of robotic mechanotherapy in the treatment of patients:

- “Locomotor training system” (for training and creating the correct walking stereotype, analogue of “Lokomat”);

- Armeo mechanized complex (allows patients, using even small functional capabilities of the upper limb, to develop and strengthen locomotor and grasping functions);

- Specialized Thera-vital and Motomed exercise bikes .

In general, during the internship period, a great deal of experience was gained in diagnosing and choosing treatment tactics for severe patients.

Residents of 2 years of study in the EP “Adult Oncology” NJSC “Astana Medical University”

The clinical internship took place on the basis of the Federal State Budgetary Institution “National Medical Research Center named after N.N. Blokhin” Department of Oncology and Radiation Therapy, Federal State Autonomous Educational Institution of Higher Education Russian National Research Medical University named after. N.I. Pirogov Ministry of Health of Russia at the address Kashirskoye Shosse, 23, under the leadership of the head of the department, Professor K.K. Laktionov and the head of the academic department A.V. Egorova.

During the entire internship, a large amount of theoretical and practical material was mastered. The clinical part took place in several departments of the named hospital:

- in the department hematology under the leadership of Tumyan G.S. examined patients, participated in weekly patient rounds, reviewed patient histories, and manually diluted chemotherapy;

- in the chemotherapy department №2 under the leadership of Tryakin A.A. participated in weekly rounds of patients, studied medical histories, examined patients, and conducted chemotherapy appointments at the clinic (under the supervision of a mentor);

- in the department of abdominal oncology №3 under the leadership of Mammadli Z.Z. assisted in the “Hartmann operation, rectal extirpation with colostomy”;

- in the department of breast tumors under the leadership of Petrovsky A.V. were present at radical mastectomy operations and participated in the consultation;

- in the department of antitumor therapy №1 under the leadership of Artamonova E.V. participated in weekly patient rounds and a pathology conference on a complex case of breast disease; Lectures were attended within the framework of the program “Central and Eastern European School of Oncology (CEESO)” on the topic “Priority directions in oncology”.

During the internship, we acquired a lot of useful information and skills for diagnosing and treating cancer patients. We will try to implement the acquired knowledge as much as possible in further clinical practice.

 

Yktiyarov Ayaz Abdirahymuly, Bakhytzhan Aygerim Bakhytzhankyzy

Resident 2 years of study in the EP "Allergology and immunology for adults, children" NJSC "Astana Medical University"

The clinical internship in the direction of “Allergology and Immunology” was held with a rich program. For two weeks we were supervised by Associate Professor of the Department of Immunology, Ph.D. Pashchenko Olga Evgenievna. The first week of training was devoted to clinical immunology. On the first day, there was an introduction to the Department of Immunology of the Russian National Research Medical University named after N.I. Pirogov, her laboratories and collaborators. On this day we studied the molecular foundations of general immunology - the head of the Department of Immunology, MBF, Doctor of Medical Sciences, Professor Lyudmila Viktorovna Gankovskaya gave us a two-hour lecture. Then, Candidate of Medical Sciences, Associate Professor of the Department of Immunology Vyacheslav Viktorovich Grechenko shared his experience and theoretical materials on assessing immune status. From the very first days, we were surprised by the departments' deep knowledge of the basics of immunology. From the second day onwards, the internship took place at the clinical base of the department - in the department of clinical immunology and rheumatology of the Russian Children's Clinical Hospital (RDCH). Here Paschenko O.E. shared her many years of experience in the wedge department. immunology and rheumatology and showed us unique clinical cases of primary immunodeficiencies (PID). Also in the department, we were able to meet and talk with Doctor of Medical Sciences, Professor, leading pediatric clinical immunologist of Russia Irina Vadimovna Kondratenko. She, in turn, conducted an integrated lesson with us dedicated to the problems of diagnosing PID. She also gave us her new book “Primary Immunodeficiencies” (800 pages), which was written on the author’s personal clinical experience. In the department we were assigned to trainee doctors. With them, we were able to participate in the management of patients diagnosed with severe congenital neutropenia, chronic granulomatous disease, Kabuki syndrome, SCID, CVID and others. We were very interested in the diagnostic and therapeutic approaches of the doctors.

The second week was spent in the allergology and immunology department of City Clinical Hospital №24 and was devoted to clinical allergology. Every day we worked with serious patients. Among them, patients with diagnoses of Drug Hypersensitivity Syndrome (DRESS syndrome – Drug reaction with eosinophilia and systemic symptoms) and toxic epidermal necrolysis were remembered. We also participated in the work of the medical advisory commission for the selection of those liable for military service with a diagnosis of “Bronchial asthma” and carried out clinical and diagnostic measures to confirm suitability and unsuitability for military service.

 

Zhaisan Aizhan Bakhytbekkyzy

Resident 2 years of study in the EP " Pediatrics " NJSC "Astana Medical University"

Clinical internship took place in the pediatric department of the Children's City Clinical Hospital (CCCH) named after Zoya Alekseevna Bashlyaeva.

In the department I examined patients with various pathologies: gastroesophageal reflux with/without esophagitis, acute and chronic gastroduodenitis, gastric ulcers, Crohn's disease, celiac disease, chronic pancreatitis, dolichosigma, chronic constipation, IBS, gastrointestinal form of nutritional allergy, exacerbation of atopic dermatitis, asthma, Reactive arthritis, Alopecia, Gilbert's disease. Together with the doctors of the department, Radchenko Elena Ravilyevna, Lyudmila Viktorovna and the head of the department, Ph.D. Alexandra Sergeevna Vorobyova examined patients: collected complaints, clinical and anamnestic data, carried out an objective examination of patients, drew up examination and treatment plans, assessed the results of laboratory and instrumental examinations; Conducted an initial examination of planned patients in a short-stay hospital and assisted department doctors in maintaining medical records.

At 05/16/2024 she participated in the following conferences:

- IX Moscow City Congress of Pediatricians with interregional and international participation “Difficult diagnosis in pediatrics”;

- conference on pediatrics by Professor Zakharova “Practical aspects of pediatric practice.”

Webinars were heard during the internship: “Problems of using gadgets in childhood. The view of a psychiatrist and ophthalmologist,” “Goat’s milk in children’s nutrition.” “Bronchial asthma in children: clinical recommendations - what’s new?”, “Multimodal ways to overcome pain”, “Immunomodulators in the treatment of ARVI: pros and cons. An infectious disease specialist’s view”, “Therapy of tonsillopharyngitis and sinusitis in children”, “A frequently ill child. Pediatrician tactics in accordance with current clinical guidelines.”

Every weekday from 11:00 to 13:00 I attended a joint project of the pediatric faculty of the Russian Medical Academy of Postgraduate Education and the Children's City Hospital named after Z.A. Bashlyaeva “School of Clinical Thinking for Pediatricians.” On Saturdays from 9:00 to 12:00 I regularly participated in the “School of a Young Children’s Doctor”.

 

Residents express their deep gratitude to the management of NJSC «Astana Medical University» for the opportunity to study abroad as part of the academic mobility program, and for the acquired professional experience that will be applicable in mastering a future specialty.
