for 1-2 courses of current master's programs (*):

Application addressed to the head of the university in which he studies (signed and stamped by the head);
Application for transfer to the head of the host university;
The profile contains a supplement to the diploma of higher education (original);
Extract on the end of the period of study in the magistracy (signed by the Vice-Rector for Academic Affairs and the Civil Registry Office);
Copy of the delivery certificate for CT;
A copy of the educational grant certificate;
Copy of identity card;
Extract from the order for the award of the scholarship;
Extracts from meetings on the approval of the topic of the thesis, supervisors and consultants of all levels;
Conclusions on ethical and moral-legal assessment (for clinical experimental studies)
Photo 3 * 4 (2 dan);
Extract from the e-zine indicating subjects and current grades.

Application addressed to the head of the university where he is studying (with the head's signature and seal);
Transfer application addressed to the head of the host university;
Master's degree or residency completion certificate with attachments;
Transcript for the completed period of study in the magistracy (signed by the vice-rector for academic affairs and the office of the registrar);
A copy of the certificate of delivery of CT;
A copy of the certificate of the holder of the educational grant;
Copy of identity card;
Extract from the order on the appointment of a scholarship;
Extracts from meetings on the approval of the topic of the dissertation, scientific consultants at all levels;
Foreign Consultant Agreement;
Conclusion on ethical and moral - legal assessment (for clinical experimental studies)
